Reconnected to Health is the North East Prison’s Integrated Healthcare Service.  The service is led by Spectrum Community Health CIC and delivered in partnership with Humankind and Tees, Esk and Wear Valleys NHS Foundation Trust.   Humankind delivers the non-clinical Drug and Alcohol Recovery element of the service promoting opportunities and supporting best possible outcomes for individuals, their families and communities across North East prisons.
This role is to work as part of a team to promote peer led interventions and visible recovery for all service users across all stages of recovery from assessment and engagement through to sustained recovery.
To enable opportunities for service users, families and carers and stakeholders to engage with those with lived experience and come into contact with visible recovery.
Working to ensure effective delivery of peer led interventions and opportunities within the wider Reconnected to Health service model.

Main duties and responsibilities:
  • To enable a comprehensive offer of peer led interventions and opportunities within a designated establishment/s ensuring visible recovery and the voice of lived experience is accessible for all service users across Reconnected to Health.
  • Work collaboratively with Reconnected to Health colleagues to implement, deliver and maintain a structured mentoring programme within the establishment/s, including effective recruitment and training, risk management, supervision and support and progression for mentors across the partnership. Also linking with establishment programmes such as PIDS and Listeners.
  • Promote service user and peer led interventions, both structured and informal, to build and sustain an active peer led recovery community within the establishment/s.
  • Support service users to develop a timetable of peer led activities, groups and sessions that reflect the interests and priorities of the men/women resident in the establishment, working closely with Reconnected to Health and establishment colleagues to safely and effectively enable delivery of the timetable. This may include mutual aid groups but also mainstream activities.
  • To plan and deliver a range of events and campaigns that actively involve men/women with the establishment promoting peer led opportunities, health and wellbeing initiatives and celebrating recovery.
  • To ensure service users are informed of opportunities and choice, promoting ownership of recovery through lived experience.
  • Connect effectively with community-based opportunities such as volunteer and mentoring schemes (both substance use and mainstream) and mutual aid provision ensuring gains made in custody can be protected and further developed following release.
  • Work closely with recovery co-ordinators to identify those who may wish to become involved in peer-led, lived experience delivery and men/women in our care who may benefit from peer-led support, ensuring this is known and available to all.
  • Facilitate service user access to treatment, assets and resources to increase recovery and social capital, supporting service users and their loved ones to achieve and sustain recovery goals.
  • To exercise excellent communication skills and therapeutic skills when dealing with service users, carers/families and partner agencies.
  • To work closely and collaboratively within the multi-disciplinary team across the Reconnected to Health partnership and externally, providing advice and information to managers and other colleagues, through attendance at reviews, team meetings and via telephone conversations and MIS information.
  • To improve outcomes for children and families of service users by reducing the impact of drug and alcohol related harm on family life and promote positive family involvement in recovery.
  • To support service users to positive transition within the prison system through, were possible, planned preparation for release or transfer that promotes and protects recovery capital. This will include effective partnership working with a range of peer-led and mutual aid groups/services.
  • To promote and enable engagement with the full range of health and wellbeing opportunities within Reconnected to Health, in particular in relation to mental health, smoking cessation, pain management and blood borne virus testing, treatment and vaccination.
Key Duties:
  • Comply with all monitoring and evaluation systems, collecting and monitoring information as appropriate and provide reports when required, actively contributing to the service achieving successful service user outcomes.
  • Maintain accurate, up to date records and work in accordance with organisational guidelines and procedures.
  • Strive to reduce substance related harm to individuals and the wider community.
  • Comply with all aspects of Humankind’s Code of Conduct and adhere at all times to national/local and Humankind safeguarding policies.
  • Comply with all Prison Service requirements including safe systems of work, Prison Service orders and instructions, identified training and security procedures.
  • Provide information in accordance with Humankind’s confidentiality policy to the wider team supporting service users ensuring a holistic recovery approach.
  • Uphold data protection legislation, including both local and formally agreed information sharing protocols and service user confidentiality agreements.
  • Actively participate in supervision and provide evidence of achieving performance targets alongside other supervision requirements as detailed within Humankind’s supervision policy.
  • Seek to improve personal performance, contribution, knowledge and skills.
  • Participate in appraisal and learning and development processes.
  • Recognise personal and professional boundaries and work within Humankind’s clinical governance framework at all times.
  • The post holder will be required to work flexibly across operational sites and within an agreed number of hours to maintain appropriate service provision. This will include evening and weekend working.
  • Provide a full range of service interventions in an outreach and in-reach capacity including satellite offices and partnership co-location venues with other organisations.
  • Contribute to maintaining safe systems of work and a safe environment.
  • Comply with all relevant policies and procedures.

Person Specification - Essential criteria:
  • NVQ Level 3 Health and Social Care or above or equivalent
  • Sound understanding and competence of interventions including brief interventions MI, PSIs, CBT and ITEP
  • Knowledge of the substance misuse, models of recovery and issues relating to recovery planning
  • Experience of promoting peer-led interventions and visible recovery either through own experience or volunteering/employment.
  • Understanding of best practice frameworks NICE, DOH, NTA, NMC, Care Quality Commission frameworks & Caldicott Standards
  • Ability to carry out assessment and recovery planning and risk management plans
  • Experience of delivering 1-1 and group therapeutic interventions
  • Ability to organise own workload in response to supervision
  • Ability to keep case notes and write reports
  • Ability to work as part of a team and on own initiative
  • Ability to write reports and complete any required monitoring returns
  • Ability to maintain and work within professional boundaries

Person Specification - Desirable criteria:
  • Professional qualification e.g. substance misuse, addiction studies, counselling, housing, development, children and families, benefits/welfare advice
